"I'm on spiro, and I asked my doctor if I could be switched to bica because I was getting dehydration and muscle cramps (I actually have been experiencing these side effects, but I didn't tell her that the real reason I want to switch is because bica is far better for feminization in every way and isn't going to give me visceral fat around my stomach or potentially stunt my boob growth).

Now she just wants to lower my spiro dosage from 200mg to 100mg and have me come in for some blood tests. How can I just fucking convince her to switch me over already? I wrote a long-ass letter to her with a bunch of links to studies about bica being safe and effective for HRT, and I'm a bit disappointed that it wasn't sufficient to just convince her straight away. Do you guys have any advice for what to say/do? Should I just start self-medding in secret, or will it be super obvious on my bloodwork?"

Getting docs to prescribe bica can be hard. I haven't seen it in any authoritative guidelines so far. The closest is the Callen-Lorde guidelines in the copypasta where they have flutamide. That has proven helpful to some anons, especially if you make it clear bica is safer than flutamide.

I think the reasons would be concerns about insurance coverage, lack of experience prescribing them, and concerns about the ability of patients to manage injections.

Not recent, sadly. Levels 2 months ago were around 100 ng/ml with 2 patches 2 days after applying them (so post-peak, since they have a life of 3.5 days). Since then I've been putting them on thinner skin around my stomach rather than the thick skin on my thighs so levels should be better.
I can't get E2 measured here so no idea what my ratio is but the effects seem to speak for themselves. I have heard that E2 can get saturated to the point of it being converted to prolactin."

That's funny and sad when you can't use Callen-Lorde's own guidelines to get them to put you on bica.
Would she put you on flutamide since it's in the guidelines? And would the risk be worth getting off spiro? I haven't really looked into the risks of flutamide because there's really no reason to use it when bica is available.

Pretty much. It might still generate a little fluid but it shouldn't be doing much. It won't totally go away though.
If you start young enough you shouldn't have to worry about enlargement like the other anon says, nor cancer. The few cases of prostate cancer in mtfs seem to be in those who started older and likely already had cancer when they started.

Flutamide is basically vastly inferior to bica in every way. It has a 6 hour half-life so you have to take it like 3-4 times/day to keep your receptors sufficiently blocked, and it's waaay less healthy for your liver than even cypro. Even if you're reacting badly to spiro like I seem to be (I forgot to mention that I've been getting occasional heart palpitations and that I can't seem to focus worth a damn lately), flutamide is probably not worth it anyway because of how bad the side effect profile is.

At this point if I were forced to choose between spiro and flutamide, I would probably just start self-medding instead. Even though my endo has generally been really nice and helpful and I still want to work with her, I wouldn't even feel bad about it because it'd be a matter of preserving my physical health against outdated/incorrect advice."

Temporarily increased sex drive, potential pain around your liver, might upset your stomach.

I would advise splitting and taking a 12.5 or at least 25 mg dose in the future as the load on first pass metabolism should be lighter (even though it will still be hard on your liver over time as it has a very long half life).

If you can, check liver enzymes in the next week or so and see where you're at. You may also want to have some milk thistle although it's debatable how much that helps."

Estrogen by itself will help a great deal with suppressing T levels, and I would normally say that a sufficient amount of injected estradiol + 25-50mg/day Bica should be enough for a near total androgen blockade, but if you're planning to do a bicalutamide monotherapy the required dosage of bica will be much higher, because your testosterone levels will be higher. You'd probably need like 150mg/day bica to block your T if it's in normal cis male ranges, which I wouldn't really recommend doing because you're more likely to have some sort of adverse side effect at that dosage.

Is your goal to just have no sex hormones at all? I think that can cause issues with things like bone density if you do it for a long time period (1 year+). I wouldn't really recommend, but hypothetically I would think cypro would be better suited for that particular purpose, since you probably wouldn't need a monstrously large dose to suppress your T."

"Hey folks, I'm now 3 years on cypro 50mg and progy 8mg. I've been given the go-ahead by Nottingham GIC to start their HRT regimen once they've written to my GP but the issue of sperm storage came up and I was surprised to hear the GIC say they see pretty significant rates of people producing (some, much reduced) viable sperm after coming off blockers even after years, even cypro.

Unfortunately they say coming off of cypro for even a few months to try and see if I can produce sperm again would result in a major termination effect i.e. accelerated masculinisation including of bone structure, which puts me off the idea completely. But I do feel bad about not having been able to afford to bank sperm when I started self-medding, and what's more, it would patch things up between me and my crazy dad significantly if he knew I still had the option to have kids in future if I changed my mind.

So my question is this- Should I switch from cyproterone to bicalutamide for a period and see if sperm production returns? If so, how should I do it- changing from one blocker to the other abruptly, or continuing to take cypro for a few weeks while also taking bica, or what? Please direct biochem anon to this post if possible."

My personal advice would be to not risk any further masculinization, and to tell your dad to go fuck himself. You don't need to be a slave to his expectations, and if you've been self-medding for 3 years with no regrets there is basically a 100% chance you're trans. It's also, in my opinion, not worth it to try to mend relationships with people who are unwilling to accept you for who you really are, even though that's a pretty hard thing to hear.

>Should I switch from cyproterone to bicalutamide for a period and see if sperm production returns?

I'd keep in mind that bica takes several weeks (maybe like 4 - 6 weeks?) before it starts to fully take effect, so if you switch then I wouldn't get off of the cypro right away."

You don't have to go the injection route if you don't really want to. After a few years and if you get an orchi and are in maintenance mode I don't think injections make as much of a difference as when you're feminizing.
>>10804178
I think needing estrone from pills is a meme that isn't accurate. In the previous thread I quoted a paper that said aromatized testosterone is actually the largest source of estradiol in breast tissue. And estrone sulfate, not estrone, was cited as another major source. You'll still have a lot of estrone sulfate floating around from injections. I speculated that nuking T too much might actually bad - but I don't know if that's really true.
I would avoid taking pills with injections. Ciswomen have estrone:estradiol ratios comparable to what you get from injections.
>>10804213
YMMV but I'd consider that a minimal dose for hrt. Usually it's not enough but if sublingual wasn't working for you this will likely be an improvement.
>>10804731
If you switched to bica I'd try tapering down your cypro dose first. You should be able to go to 25 mg right away, then go to 12.5.
After this much time on hrt you should be able to taper down cypro even if you weren't switching.
Pic related shows levels of bica over time. It takes at least 2 weeks to get to decent levels. After a month you're almost as high as you're going to get."

>>10806974
If you want to go on raloxifene that might be why they want you to talk to a social worker. Informed consent normally applies to full feminizing hrt, and they want you to consent to the full package, including breasts. Raloxifene shows that you're reluctant to consent to full hrt, hence the social worker.
There are issues with raloxifene causing increased testosterone levels. And it's not a full replacement for estradiol so you're still vulnerable to health effects from low hormones if you're taking antiandrogens.
But they only prescribe spiro and spiro is going to be really shitty when taken with ralox. Normally ralox would be taken with bica because bica is a good blocker and you get excess T converting to estradiol. But it seems that bica+ralox is of dubious effectiveness for feminizing, especially if you're not taking cancer doses of bica to block all the T totally.
But anyway almost no one prescribes bica. And IC places are going to be reluctant to do femboy-type stuff.
>>10807467
Osteoporosis, low energy, depression, apathy, dry and deteriorating skin, hot flashes, night sweats. YMMV but it can be nasty.
Taking AAs only can be worse for your health than getting an orchi and not taking estradiol because you're blocking the androgen receptors and weakening the effects of adrenal androgens.

There's always the orchi option then you can get off AAs.
Injections + a progestin might be enough to downregulate T production to where you can go off AAs. No guarantee.
GnRH agonists are an option if you can afford them.
If you're at the point of going repgen or the noose you can try something like Diane-35 if you're willing to risk blood clots. There's a small amount of cypro in them, and ethinyl estradiol. Ethinyl estradiol is good at downregulating T production, plus you get help from the cypro. But the blood clot risk is significant and EE can be hard on the liver too."

Otherwise lower sex drive, change in body odor, possible drop in energy levels can be noticed quickly. Skin changes and slowing of hair growth too. But you might already be experiencing these.
>>10809783
Generally yes but you should do it in moderation of course.
>>10810450
Depends on what you're taking. Regardless tapering off is better than going cold turkey. But be careful about stopping since you'll regret it if you decide to restart.
>>10810584
You can boymode and at least you won't age like a man. Even if you don't look exactly the way you'd like, by stopping you can end up looking like something worse. And you don't want to crack when you're 40 and become a gigahon.
Just make sure you know what you're doing and make sure you're not holding yourself to too high a standard.

You ought to taper off the spiro. Going cold turkey might throw off your electrolytes.
I don't have any authoritative formula for the best way to taper off unfortunately. It also depends on how much of a hurry you're in. You could go slow and drop spiro 50 mg at a time, maybe taking a week between each dose change. Or go 200, 100, 50 over three weeks if you're in a hurry. If you feel bad side effects you can slow it down. You could drop your E to 2 mg right away, or wait until you're halfway through tapering spiro. Maybe when you're at 50 or 0 spiro you could drop the E.
But this is being cautious. Realistically you could probably do it a lot faster. And it depends on your tolerance of side effects."

If you've been on it for a while you're going to keep some of those effects for good, but also if you've been on it for a while then clearly there was a reason you did that in the first place, and if you're anything like me that reason isn't going to suddenly disappear (like I hoped it would). On hrt your fat distribution will be more female, which you would want? How do you feel about aging going forward? And what would help you get the kind of man you want?
>>10810934
Are you on anything right now? Do you have test results? Low baseline T makes it easier to suppress it, but unless you have a baseline in the female range you can't be sure what it will take to get it down to that point. The safest thing is to start with AAs then taper them off and see if T levels stay acceptably low.
>>10810980
I'd stay on it. It seems like you don't question whether you're trans, just whether you'll get the results you want. Being trans, the only alternative to hrt is repression, and repression has a poor track record in terms of happiness and being able to stick with it. Cycling on and off hrt is the worst thing you can do. You also don't want to start repressing then crack when you're 40 or something. Unless you're sure that aging as a man with male testosterone levels until you die is the course you're going to follow, and that it's the best course, then you should stay on hrt and boymode if you have to. Even if hrt doesn't do everything you'd want it to do, it can still be better than not being on it, which makes it worth it.
>>10811061
Since you don't dislike conetits that's a strong sign that you're better off on hrt. If growing boobs is something that you hated and was upsetting you then maybe you'd have grounds to stop. But like you said, it looks like you're stuck on them.

Getting skin changes without breast growth is impossible. You either take the whole package or you take nothing at all. At one month, you are unlikely to get any visible effects yet - you'll just have reduced libido and body odor. The onset of skin, fat and muscle changes caused by HRT comes sometime after the 3rd month usually, and the changes take place over the course of years. If you don't want breast growth and are worried about "damaging your body" (I assume you mean your fertility) you shouldn't touch HRT at all. It's not the right thing for you.

It would stand to reason that it would help mitigate the effects, although there hasn't been, to my knowledge, any studies confirming it. The closest I can think of is a study that showed dut reduced the symptoms of PMDD by reducing the progesterone metabolites which occur in excess during that stage of the menstrual cycle. So adding P when you are deficient should help.

It usually does, the exception being if the reason for being unattractive as a guy is from feminine characteristics, but otherwise conventionally attractive/aesthetic. "Too fem" 'guy'+hrt = qt.

If it's some asymmetry or androgenized borderline acromegaly hrt won't fix it. For example if someone is unattractive in boymode due to a large nose then RIP. But if it's due to a weak chin, baby face, narrow shoulders, short, etc., then you're golden."

This isnt a forum full of medical doctors. It's a collection of people who come to one place to ask questions, and when you ask a question chances are there might be another question that was asked that you can answer. When someone answers you, you can now answer if someone asks the same question in the future. Thus knowledge is shared.

The very fact that you are here asking a question, which you dont know the answer to, should be evidence that not everyone else knows the answer either and you need to wait for someone who does to come along."

The meningioma risk is fairly low if you're on 25 mg or less. The lower the better. It was thought the risk was basically nil at 25 or below, but there's some evidence that's not the case. Still you should use the lowest possible dose.
Prolactinomas are fairly uncommon too I think, though elevated prolactin is common. The exact cause/effect relationships for prolactinomas aren't fully worked out.

Breast sensitivity can be a combination of tingliness, soreness, and pain. Expect breasts to be more sensitive to the touch and easier to hurt. It's not as bad as getting kicked in the nuts but the pain can be pretty bad if you take a blow to the boobs.
Sports bras and binders can help you keep boobs from being noticeable if they grow a lot. Dark, baggy clothes are standard. If you can order stuff online without getting your packages inspected you can do that. Or you can get someone to buy one for you, or work up the courage to do it yourself. You'll have to get used to wearing and buying them unless you end up really flat."

What dose were you getting? The 5 mg dose is marketed for prostate enlargement. 1 mg is for hair loss. I could see them not covering 5 mg, but if you have diagnosed hair loss you should be able to get 1 mg covered.
Generics should be pretty cheap if you shop around though.

Asymmetry of the face is bad for attractiveness. I'm not sure it means anything in particular because it's not uncommon to have asymmetry in development. But it's not attractive in the face. Sometimes asymmetry lessens over time since development can happen at different speeds. But starting hrt in your late teens/early 20s can stop masculine growth and you might be stuck with that. But I'd think it's better than everything being masculine.

Cypro alone for a month won't hurt anything, except that if you're trying to regain fertility that's going to mess you up.
I doubt the hair loss is anything permanent. It's probably just hair growth cycles resetting - telogen effluvium. What you need is to get back on estradiol. I don't know if cypro alone will affect things. I wouldn't count on it.

Doctors tend to be hyper-busy and they often get behind on paperwork. Friendly pestering rather than complaints is probably a better way to go. You just have to keep after them. Odd the 1 mg tablets are more expensive according to goodrx.com. 30x5 is on the Walmart discount list, but the 1 mg tablets aren't on there at all. So the best thing is to get a prescription for 5 mg and just pay cash. You could take the whole thing, or split it in halves or quarters since it's almost as effective as 5.

"I just had the stupidest doctor's visit ever:

>Used to be on 4mg oral estradiol, my E2 levels were 76 pg/mL
>Asked to be switched over to 10mg IM estradiol valerate / week after my last doctor's visit
>Get bloodwork done for the first time after switching to injections
>E2: 1,048 pg/mL
>My endo is STILL trying to tell me that there is no difference between how well oral and injected medications work

>I suggest to her that I might consider switching to injecting 5mg every 3.5 days because I've been getting mood swings by the 6th and 7th day and it might smooth out the peaks/troughs of my blood levels
>She tells me that I shouldn't 'make my life difficult' by injecting so often, and suggests instead that I could switch to doing it once every 2 weeks so I don't have to inject so often
>I explain that since estradiol valerate has a 5 day half-life, there would be an extreme difference between my peak and trough levels and that would probably be not very good for feminization
>She tries to tell me that 5 days doesn't sound right to her

>Ask her about potentially switching from spironolactone to literally anything else, because I'm getting sides that are making me uncomfortable (dehydration, muscle cramps, etc.). I bring up bica as an alternative
>Instead of switching me to anything better, she just reduces my spiro dosage from 200mg to 100mg

What the fuck is my endo doing? Is she an idiot, or is she intentionally trying to give me misinformation for god-knows-what reason? Is she secretly some kind of spironolactone cultist? I don't even know which scenario would be more horrifying to me. This doctor's visit was NOT cheap, either.

Anyway, I'm just going to self-med bica starting now. My T levels for my first two checkups have been 22ng/dL and 26ng/dL respectively, so a 25mg/day dose should be sufficient, right? Do you think my T levels would rebound at all from quitting the spironolactone, or are my crazy high E2 levels probably going to keep it in check?"

I can understand that there are limitations to what doctors can prescribe because if they do anything unconventional and it somehow goes wrong, they could potentially get in a lot of trouble with medical malpractice suits or whatever, or (more likely) their bosses will just get pissed at them for breaking the standard code for how things are done. To be fair doctors have to have pretty broad knowledge of a wide range of conditions. And they're usually going to be too busy and tired to spend a lot of time researching things on their own. It's safe to just stick with existing guidelines.
Let's hope that bica starts making it into official guidelines so that US mtfs will have an affordable legit alternative to spiro."

Forgot to answer. With those T levels 25 mg of bica should be enough to block it. And the high E levels from injections will help keep it down. However I don't know how well it will stay down. You'll want to check your T level to make sure it doesn't go up again.
That said there's more than one study showing spiro overall isn't that helpful for lowering T. But it does seem to help some people a lot. So who knows. You'll just have to try and see what happens.
If T levels start getting above the female range you might want to bump up to 50 mg/day. If you got up into the male range you'd definitely want to go to 50."

This is a very complicated subject and research still needs to be done.
https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2911511/ is a decent though technical article about nuclear receptor regulation.
In some cells you get autoinduction where there's positive feedback. This is mostly in liver, pituitary, and reproductive tract cells.
You can also get autorepression, with negative feedback. Unfortunately this happens in breast glandular cells.
Glucocorticoids like cortisol can downregulate estrogen receptors in breasts and bones.
This article https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2889220/
talks about estradiol and fat cells.
In this article https://www.ncbi.nlm.nih.gov/pubmed/21177016 which looked at embryonic stem cells, supraphysiologic levels of estradiol downregulated HDAC1, an important enzyme involved with cell proliferation. I don't know exactly what it does but it seems important.
All of this is insanely complicated and it looks like a lot of details need to be worked out. The best thing I can say is that it's probably best to have normal female levels of estradiol. Too much seems like it can be bad in some cases."

If you DIY patches they won't be able to tell unless they check for estrone levels, which I doubt they will do. You'll be messing with their clinical experience though, making docs think pills are better than they really are.

The patches without insurance cost a fortune so I went with the cheap pills. At least she it'd be like 10 bucks Vs 300 (more like 30 for the pills but whatever). Maybe she lied to me to nerf my transition.

Should I just swallow the pills orally? A lot of times sublingual I have to talk on the phone or whatever and the mushy dissolved stuff just goes down my throat... Maybe that's why because it doesn't work as well if it's mushed up Vs whole if swallowed.

I'm on 6mg so I dunno wtf is even going on.... Wut. 350 to 91 what the fuck happened. I dunno. My heads swirling... Ummm fuck me. Shit I gotta do another appt in a bit. I hate my life....

I want to do things the right way but everytime I do I'm punished. Fuck it. Fuck being honest and doing the system. Fuck it. The system sucks. You get a smidgen of time then they lie to you and nerf what you've accomplished so they can laugh at you. Just diy. What a waste of 136 fucking dollars.... Oh well I'll have useless Hon pills to stockpile like a retard. I'm so stupid and make the worst possible decision constantly about everything."

OK, thanks for the help. I do already have fair-sized breasts. I'd at least like to switch to bica for my AA if I do continue with estrogen (and I think I probably will). I'm just having an identity crisis and need to know all my options. thanks anon."

>Not good. Can you just say you ordered some things over the internet without telling them what you ordered? It really isn't any of their business what you're spending your money on.

First time I ordered, I did the same thing and transfered money. The only reason I did it again was because they stopped accepting BTC.
The last time I got a call from my bank and they were very pushy to know what I had paid for and I told them it was beauty products such as face creams and stuff. This time it's going to be a meeting face to face and I feel like I can't bail if they try to force me to tell them what it was."

Ugh, dont swallow it it lowers your dose. Lie you tongue flat on the bottom, and dont talk. Dont chew it, just place it under your tongue and let it dissolve and sit there tongue it moving until there is no taste. Anything less and you are wasting estrogen. I uasually do it on the toilet or while playing hearthstone on my phone."

Swallowing them is of course not going to work as well so if you're swallowing the pills a lot that could be part of the problem. I doubt just swallowing them whole will help. You could try taking them 3 times a day, and split the pills before you put them under your tongue so they dissolve faster. That should give you more consistent levels."

That sounds bad. I don't know what the law is there so I don't know how much the bank is entitled to know or what they'd do with the information, or what kind of privacy rights you might have.
If you survive this it does sound like you're going to have to find another way to pay. You don't want to go through all this again."

Sweden isn't that strict when it comes to this usually but it's a scary situation. I need to come up with something believable to tell them."

Is the government involved in this or just the bank? How illegal is it to order mtf hrt? Usually the answer is not very or even not at all.
I don't know the law over there though. Are you under any legal obligation to say anything? It might be best just to not say anything. Here in the US falsifying paperwork or lying to authorities could get you in a lot more trouble than ordering hrt would.
You probably need a lawyer if the authorities are now or might become involved.

Thanks. I hope it helps. But if not the other option would be injections if you can tolerate the needles. Injections should cost a lot less than patches if you go with generic estradiol valerate. That's not as good as estradiol cypionate but it's cheap, especially if you get the more concentrated vials. The concentrated injections are good for subcutaneous use too since you only have to inject a small amount of fluid at any one time."

It's just their bank asking my bank what the money is for, the problem comes from my bank pushing me to tell them exactly what I've ordered."

Seems odd that IHP's bank is initiating this. Maybe it's IHP that's in trouble!
I really don't know what you should do to be honest. Here in the US I wouldn't get in trouble for ordering hrt, so if I couldn't get away with telling them it's none of their business, I'd just tell the truth. It wouldn't be worth it to lie about something I wouldn't get in trouble for anyway. At least if they know it's hrt then they know it's nothing nefarious, and it would be understandable if you've been reluctant to disclose it."
