Getting docs to prescribe bica can be hard. I haven't seen it in any authoritative guidelines so far. The closest is the Callen-Lorde guidelines in the copypasta where they have flutamide. That has proven helpful to some anons, especially if you make it clear bica is safer than flutamide.
If you self med it probably will be obvious due to high T levels.
Bica is probably best for that. Some anons have said spiro killed their dick more than cypro.
500 mIU/L / 23.5 ng/mL is the usual limit of normal. 1000 mIU/L / 47 ng/mL is where you start getting concerned and want to find the cause and bring the levels down. 5000 mIU/L / 235 ng/mL is where you probably have a prolactinoma and you need to be very worried and get it fixed.
If you're not lactating you're probably not at panic levels, but you can't totally rely on that."

I think the reasons would be concerns about insurance coverage, lack of experience prescribing them, and concerns about the ability of patients to manage injections.
If you could convince the doctor you could handle injections, and make arguments about having a lower blood clot risk and better estrone:estradiol ratio than other methods that could help.

Not recent, sadly. Levels 2 months ago were around 100 ng/ml with 2 patches 2 days after applying them (so post-peak, since they have a life of 3.5 days). Since then I've been putting them on thinner skin around my stomach rather than the thick skin on my thighs so levels should be better.
I can't get E2 measured here so no idea what my ratio is but the effects seem to speak for themselves. I have heard that E2 can get saturated to the point of it being converted to prolactin."

If you start young enough you shouldn't have to worry about enlargement like the other anon says, nor cancer. The few cases of prostate cancer in mtfs seem to be in those who started older and likely already had cancer when they started.
I guess you can if you have to, but I think bioavailability is better with food.
I'd say it can since androgen receptor blockage would kick in quickly. And it can nuke T production in a month or less too.

Just deal with it and you will eventually get used to it, drink caffeinated drinks if you need to. Also take the pill after dinner because it has a bit of a sedative effect. It doesn't just happen with cypro though. Blocking testosterone will generally make you less energetic and extremely tired for a while. You eventually get used to having less energy. Funnily enough it's the opposite for FTMs."

Flutamide is basically vastly inferior to bica in every way. It has a 6 hour half-life so you have to take it like 3-4 times/day to keep your receptors sufficiently blocked, and it's waaay less healthy for your liver than even cypro. Even if you're reacting badly to spiro like I seem to be (I forgot to mention that I've been getting occasional heart palpitations and that I can't seem to focus worth a damn lately), flutamide is probably not worth it anyway because of how bad the side effect profile is.

At this point if I were forced to choose between spiro and flutamide, I would probably just start self-medding instead. Even though my endo has generally been really nice and helpful and I still want to work with her, I wouldn't even feel bad about it because it'd be a matter of preserving my physical health against outdated/incorrect advice."

Temporarily increased sex drive, potential pain around your liver, might upset your stomach.

I would advise splitting and taking a 12.5 or at least 25 mg dose in the future as the load on first pass metabolism should be lighter (even though it will still be hard on your liver over time as it has a very long half life).

Estrogen by itself will help a great deal with suppressing T levels, and I would normally say that a sufficient amount of injected estradiol + 25-50mg/day Bica should be enough for a near total androgen blockade, but if you're planning to do a bicalutamide monotherapy the required dosage of bica will be much higher, because your testosterone levels will be higher. You'd probably need like 150mg/day bica to block your T if it's in normal cis male ranges, which I wouldn't really recommend doing because you're more likely to have some sort of adverse side effect at that dosage.

Is your goal to just have no sex hormones at all? I think that can cause issues with things like bone density if you do it for a long time period (1 year+). I wouldn't really recommend, but hypothetically I would think cypro would be better suited for that particular purpose, since you probably wouldn't need a monstrously large dose to suppress your T."

I've been given the go-ahead by Nottingham GIC to start their HRT regimen once they've written to my GP but the issue of sperm storage came up and I was surprised to hear the GIC say they see pretty significant rates of people producing (some, much reduced) viable sperm after coming off blockers even after years, even cypro.

Unfortunately they say coming off of cypro for even a few months to try and see if I can produce sperm again would result in a major termination effect i.e. accelerated masculinisation including of bone structure, which puts me off the idea completely. But I do feel bad about not having been able to afford to bank sperm when I started self-medding, and what's more, it would patch things up between me and my crazy dad significantly if he knew I still had the option to have kids in future if I changed my mind.

So my question is this- Should I switch from cyproterone to bicalutamide for a period and see if sperm production returns? If so, how should I do it- changing from one blocker to the other abruptly, or continuing to take cypro for a few weeks while also taking bica, or what? My personal advice would be to not risk any further masculinization, and to tell your dad to go fuck himself. You don't need to be a slave to his expectations, and if you've been self-medding for 3 years with no regrets there is basically a 100% chance you're trans. I tried buccal because it's supposed to be better than sublingual but I had the same problem. Too slow, and that slurry was irritating my gums pretty badly.
Splitting pills in half helps them dissolve more quickly.

If you're not taking E you might need finasteride or dutasteride to save hair. It varies by individual.
Like the other anon says you might need up to 150 mg/day with bica alone. That's the cancer dose.

You don't have to go the injection route if you don't really want to. After a few years and if you get an orchi and are in maintenance mode I don't think injections make as much of a difference as when you're feminizing.
I think needing estrone from pills is a meme that isn't accurate. In the previous thread I quoted a paper that said aromatized testosterone is actually the largest source of estradiol in breast tissue. And estrone sulfate, not estrone, was cited as another major source. You'll still have a lot of estrone sulfate floating around from injections. I speculated that nuking T too much might actually bad - but I don't know if that's really true.
I would avoid taking pills with injections. Ciswomen have estrone:estradiol ratios comparable to what you get from injections.
YMMV but I'd consider that a minimal dose for hrt. Usually it's not enough but if sublingual wasn't working for you this will likely be an improvement.
If you switched to bica I'd try tapering down your cypro dose first. You should be able to go to 25 mg right away, then go to 12.5.
After this much time on hrt you should be able to taper down cypro even if you weren't switching.

If you want to go on raloxifene that might be why they want you to talk to a social worker. Informed consent normally applies to full feminizing hrt, and they want you to consent to the full package, including breasts. Raloxifene shows that you're reluctant to consent to full hrt, hence the social worker.
There are issues with raloxifene causing increased testosterone levels. And it's not a full replacement for estradiol so you're still vulnerable to health effects from low hormones if you're taking antiandrogens.
But they only prescribe spiro and spiro is going to be really shitty when taken with ralox. Normally ralox would be taken with bica because bica is a good blocker and you get excess T converting to estradiol. But it seems that bica+ralox is of dubious effectiveness for feminizing, especially if you're not taking cancer doses of bica to block all the T totally.
But anyway almost no one prescribes bica. And IC places are going to be reluctant to do femboy-type stuff.

Taking AAs only can be worse for your health than getting an orchi and not taking estradiol because you're blocking the androgen receptors and weakening the effects of adrenal androgens.

There's always the orchi option then you can get off AAs.
Injections + a progestin might be enough to downregulate T production to where you can go off AAs. No guarantee.
GnRH agonists are an option if you can afford them.
If you're at the point of going repgen or the noose you can try something like Diane-35 if you're willing to risk blood clots. There's a small amount of cypro in them, and ethinyl estradiol. Ethinyl estradiol is good at downregulating T production, plus you get help from the cypro. But the blood clot risk is significant and EE can be hard on the liver too."

No, it's a SERM. It only has limited estrogenic effects. It helps with osteoporosis and fat distribution, but not as well as estradiol. There is a blood clot risk.
It's hard to know without knowing where you're at right now in terms of T levels and effects. If you're already noticing the effects of estradiol you probably won't notice that much. Otherwise lower sex drive, change in body odor, possible drop in energy levels can be noticed quickly. Skin changes and slowing of hair growth too. But you might already be experiencing these.

You ought to taper off the spiro. Going cold turkey might throw off your electrolytes.
I don't have any authoritative formula for the best way to taper off unfortunately. It also depends on how much of a hurry you're in. You could go slow and drop spiro 50 mg at a time, maybe taking a week between each dose change. Or go 200, 100, 50 over three weeks if you're in a hurry. If you feel bad side effects you can slow it down. You could drop your E to 2 mg right away, or wait until you're halfway through tapering spiro. Maybe when you're at 50 or 0 spiro you could drop the E.
But this is being cautious. Realistically you could probably do it a lot faster. And it depends on your tolerance of side effects."

Are you on anything right now? Do you have test results? Low baseline T makes it easier to suppress it, but unless you have a baseline in the female range you can't be sure what it will take to get it down to that point. The safest thing is to start with AAs then taper them off and see if T levels stay acceptably low.
I'd stay on it. It seems like you don't question whether you're trans, just whether you'll get the results you want. Being trans, the only alternative to hrt is repression, and repression has a poor track record in terms of happiness and being able to stick with it. Cycling on and off hrt is the worst thing you can do. You also don't want to start repressing then crack when you're 40 or something. Unless you're sure that aging as a man with male testosterone levels until you die is the course you're going to follow, and that it's the best course, then you should stay on hrt and boymode if you have to. Even if hrt doesn't do everything you'd want it to do, it can still be better than not being on it, which makes it worth it.
Since you don't dislike conetits that's a strong sign that you're better off on hrt. If growing boobs is something that you hated and was upsetting you then maybe you'd have grounds to stop. But like you said, it looks like you're stuck on them.

At one month, you are unlikely to get any visible effects yet - you'll just have reduced libido and body odor. It would stand to reason that it would help mitigate the effects, although there hasn't been, to my knowledge, any studies confirming it. The closest I can think of is a study that showed dut reduced the symptoms of PMDD by reducing the progesterone metabolites which occur in excess during that stage of the menstrual cycle. So adding P when you are deficient should help.

You can't really know without testing I'm afraid.
Maybe a social worker is standard at this place. It's not uncommon to have social workers available, though usually they're optional.
The meningioma risk is fairly low if you're on 25 mg or less. The lower the better. It was thought the risk was basically nil at 25 or below, but there's some evidence that's not the case. Still you should use the lowest possible dose.
Prolactinomas are fairly uncommon too I think, though elevated prolactin is common. The exact cause/effect relationships for prolactinomas aren't fully worked out.

Sports bras and binders can help you keep boobs from being noticeable if they grow a lot. Dark, baggy clothes are standard. If you can order stuff online without getting your packages inspected you can do that. Or you can get someone to buy one for you, or work up the courage to do it yourself. Sometimes asymmetry lessens over time since development can happen at different speeds. But starting hrt in your late teens/early 20s can stop masculine growth and you might be stuck with that. But I'd think it's better than everything being masculine.

They're cheap. 30 5 mg tablets are $9 at Walmart. If you can't afford that you have bigger problems.
Odd the 1 mg tablets are more expensive according to 30x5 is on the Walmart discount list, but the 1 mg tablets aren't on there at all. So the best thing is to get a prescription for 5 mg and just pay cash. You could take the whole thing, or split it in halves or quarters since it's almost as effective as 5.
A lot of times referrals are done by computer, and you might need insurance approval if that comes into play.

Forgot to answer. With those T levels 25 mg of bica should be enough to block it. And the high E levels from injections will help keep it down. However I don't know how well it will stay down. You'll want to check your T level to make sure it doesn't go up again.
That said there's more than one study showing spiro overall isn't that helpful for lowering T. But it does seem to help some people a lot. So who knows. You'll just have to try and see what happens.
If T levels start getting above the female range you might want to bump up to 50 mg/day. If you got up into the male range you'd definitely want to go to 50."

Breast development is usually somewhat uneven. And for some reason the left breast usually develops faster and ends up slightly larger.
Are you only on spiro? Not estradiol too? Seems odd you'd have high prolactin levels if you aren't on estradiol.
Splitting it into more doses should be better since it will even out the spikes. I've started doing 1 mg 4 times a day. Splitting pills helps them dissolve faster. But taking them 4x a day is getting kind of annoying, and it's not possible to do it exactly 6 hours apart. I think I feel better doing it like this, but that could be placebo.
Get a pill cutter or switch to a different brand or a generic that's scored for easy splitting. The Teva pills are really good imho.

It can work but estradiol levels are likely to be suboptimal, and T levels could get really high. 50 mg might not be enough, and if you go higher the health risks increase. You haven't mentioned anything about breasts either but you'd still probably grow them. Bone density seems OK on bica alone.
I use the same pills. I've been happy with them and get better levels with 4 mg, though I've had variation. Patches are better though. I'm surprised PP wouldn't prescribe patches because they are generally considered to be better and safer. And when I used patches I did get better breast growth than with anything else I used. I don't care now that I'm done with breast growth though.
If you DIY patches they won't be able to tell unless they check for estrone levels, which I doubt they will do. You'll be messing with their clinical experience though, making docs think pills are better than they really are.
