1. Large open fire in area filled with monsters, previously they always found a secluded easily defendable area.
2. Huge group of adventurers in one spot, dungeon reacts heavily against large groups.
3. Party leaders all froze up so the counter attack was disorganized.
4. When the leaders began reacting some of them they gave bad orders, subdue instead of kill.
5. Previous inter party conflict resulted in two of the leaders being weakened.

The battle could have been won but it could have more easily been avoided. The stairs going up to a safer level were right there but they got distracted.'

I don't see Shuro bringing his retainers simply because he wouldn't have had the chance to in this scenario.

Falin is bad at communicating her desires and lacks assertiveness. Everyone thought they agreed not to rescue Laios, so they had no idea Falin would go in. She then snuck in by herself. They'd notice she's missing later.
As to why they didn't walk her home or something after her brother died, this is during the beginning of the story, when everyone was quite distant. Big monster is a great wyrm blue dragon sleeping on its hoard, and basically they have to sneak in, rescue the magic captive who can guide them to their destination, and steal a magic dagger with an ancient key hidden in the hilt that lets them access it. Party has to split up, with the group going after the dagger going into the dragon's lair. There is a sort of alarm enchantment on the horde but it can be countered or bypassed; I just wanted to emphasize stealth and make them think outside the box for a change.'
